2026 08 04 en seeing a doctor us Travel English |

Amy

Hey Jason, I’ve had a sore throat and a fever since last night.

Jason

How long have you had the fever? Is it high, or any trouble breathing?

Amy

Not too high, and I’m breathing fine. My throat just really hurts.

Jason

Okay, then you don’t need the ER. That’s for real emergencies.

Amy

Right, and it’s crazy expensive too.

Jason

Exactly. Try urgent care instead.

Amy

Do I need an appointment?

Jason

Nope, it’s walk-in. Way faster and cheaper than the ER.

Amy

Good, I’m worried about the bill. Is it covered by my insurance?

Jason

Probably. Just ask, “Do you take my insurance?” when you check in.

Amy

What if they’re out-of-network?

Jason

Then you’d pay a lot more — always check first.

Amy

What’s a copay, again?

copay(掛號自付額)到底是什麼來著?

Jason

The flat fee you pay per visit. Ask what yours is too.

Amy

Got it. What about my regular doctor?

Jason

Save that for a check-up. Urgent care’s faster for this.

Amy

Makes sense. Heading over now — thanks, Jason.

Jason

Anytime. If they give you a prescription, grab it at the pharmacy after.

Amy

Will do. Text you later!

Jason

Sounds good. Feel better soon!

Vocabulary Boost

primary care doctor / PCP/ ˈpraɪmeri kɛr ˈdɑktər / n.

I need to see my primary care doctor for a check-up.(我要去看家庭醫師做健康檢查。)

urgent care/ ˈɜrdʒənt kɛr / n.

Let’s just go to urgent care instead of waiting for an appointment.(我們直接去急症診所吧,不用等預約。)

emergency room / ER/ ɪˈmɜrdʒənsi rum / n.

If you can’t breathe, go straight to the ER.(如果你無法呼吸,直接衝急診。)

copay/ ˈkoʊpeɪ / n.

What’s my copay for an urgent care visit?(我去急症診所要付多少掛號自付額?)

deductible/ dɪˈdʌktəbəl / n.

I haven’t hit my deductible yet, so I’m paying most of it myself.(我還沒達到自付額門檻,所以大部分都是自己付。)

in-network/ ɪn ˈnɛtwɜrk / adj.

Make sure the clinic is in-network before you go.(去之前先確認這家診所有沒有跟你的保險簽約。)

prescription/ prɪˈskrɪpʃən / n.

The doctor gave her a prescription for antibiotics.(醫生開了一張抗生素的處方箋給她。)

symptom/ ˈsɪmptəm / n.

What symptoms have you had?(你有哪些症狀?)

appointment/ əˈpɔɪntmənt / n.

I’d like to schedule an appointment for next week.(我想預約下週的看診時間。)

walk-in/ ˈwɔk ɪn / n. / adj.

Do you take walk-ins, or do I need an appointment?(你們接受免預約嗎,還是我需要先預約?)

Sentence Patterns

1. I have a ___ → 我有 ___(症狀)

I have a sore throat.(我喉嚨痛。)
例:I have a fever.(我發燒。)
例:I have a runny nose.(我流鼻水。)

2. It hurts when I ___ → 我一 ___ 就會痛

It hurts when I swallow.(我一吞東西就痛。)
例:It hurts when I move my arm.(我一動手臂就痛。)
例:It hurts when I breathe deeply.(我一深呼吸就痛。)

3. How long have you ___?(現在完成式)→ 你這個狀況多久了?

How long have you had this fever?(這個燒多久了?)
例:How long have you had a cough?(咳嗽多久了?)
例:I’ve had it since last night.(我從昨晚開始就這樣了。)
回答時用 since(從某個時間點)或 for(一段時間)都可以,例如 since last night 或 for two days。

4. Do you take ___?→ 你們有接受 ___ 嗎?

Do you take my insurance?(你們有接受我的保險嗎?)
例:Do you take walk-ins?(你們接受免預約嗎?)
例:Do you take cash?(你們收現金嗎?)

Is this covered by my insurance?(這個有理賠嗎?)
例:Is the visit covered?(這次看診有包含在內嗎?)
例:Is this medication covered?(這個藥有理賠嗎?)

J
Quick quote to kick things off: “I’ve had a sore throat and a fever since last night.” I’ve had a sore throat and a fever since last night.”(我從昨晚開始喉嚨痛又發燒)。That’s Amy, calling a friend for help, a little worried. I’m Jason, and welcome back to MJ English.
M
And I’m Mary!我是 Mary!Today we’re talking about something every foreigner in America eventually stresses over — figuring out where to actually go when you’re sick, without accidentally getting hit with a giant bill.
J
Right, because in the U.S., “go see a doctor” isn’t one single option. There’s your primary care doctor, urgent care, the emergency room, even telehealth.有 primary care doctor(家庭醫師)、urgent care(急症診所)、emergency room(急診),甚至還有視訊看診。Pick the wrong one and it can cost you hundreds, even thousands.
M
Let’s start with the basics. Primary care doctor — PCP — is your regular family doctor. Check-ups, ongoing conditions, anything that isn’t urgent. But almost every visit needs an appointment first.
J
So PCP is for planned stuff, not “I feel terrible right now.” Amy’s fever hit suddenly overnight — calling her PCP and waiting three days for an appointment obviously wasn’t going to work.
M
Exactly.對。That’s where urgent care comes in. It’s walk-in — no appointment at all. Perfect for things that are annoying but not dangerous: fevers, sprains, minor infections, the flu.
J
Wait — so I can just show up, no calling ahead?
M
Pretty much. You might wait a bit if it’s busy, but there’s no booking involved. That’s the whole point — urgent care exists for exactly this kind of “I need help today” moment.
J
And here’s the part that actually saves you money — urgent care is way cheaper than the ER for the exact same problem. Same sore throat, completely different bill depending on which door you walk through.
M
Then there’s the emergency room — ER for short. That’s strictly for real emergencies: chest pain, heavy bleeding, trouble breathing, anything that could genuinely be life-threatening. Not a sore throat, not a mild fever.
J
Wait — so what’s the simple rule here?
M
Easy rule: not urgent, not dangerous, goes to urgent care. Chest pain, can’t breathe, might be life-threatening — that’s ER, no hesitation. And when you’re not sure, urgent care staff can always send you to the ER themselves.
J
There’s also telehealth now — a video call with a doctor for something small, like a rash or a quick question. Sometimes that alone means you don’t even need to leave the couch.
M
Okay, now the part everyone actually stresses about — insurance. Before any of this, you need your insurance card, and a few key words to survive the conversation.
J
First word: copay.第一個字:copay(掛號自付額)。That’s the fixed fee you pay per visit — maybe twenty-five dollars for a PCP visit, more for urgent care, a lot more for the ER.
M
Second: deductible.第二個:deductible(自付額門檻)。That’s how much you pay out of your own pocket before your insurance really starts covering things. Hit your deductible, and suddenly insurance pays a much bigger share.
J
So say your deductible is a thousand dollars. Until you’ve paid a thousand out of pocket for the year, you’re covering most costs yourself. After that, insurance steps in a lot more.
M
Third one trips up almost everyone — in-network versus out-of-network.第三個幾乎每個人都會卡到——in-network(有簽約)跟 out-of-network(沒簽約)。In-network means the place has a deal with your insurance, so it’s much cheaper. Out-of-network can cost double, or not be covered at all.
J
So before you even walk in anywhere, ask two things: “Do you take my insurance?” and “What’s my copay?” Do you take my insurance?”(你們接受我的保險嗎?)跟 “What’s my copay?”(我的掛號自付額是多少?)。Thirty seconds on the phone can genuinely save you hundreds of dollars.
M
And if you’re ever unsure something’s included, just ask directly — “Is this covered?” Don’t assume. American medical bills are famous for surprises nobody saw coming.
J
That’s basically what Amy does with her friend. She asks, “Is it covered by my insurance?” and he tells her to just call and ask flat out. Is it covered by my insurance?”(我的保險有理賠嗎?),朋友就叫她直接打電話問清楚。Smart — guessing wrong here gets expensive fast.
M
Once you’re actually being seen, you’ll need to describe your symptoms. Simple pattern — “I have a sore throat,” “I have a fever,” “I have a runny nose.”句型很簡單——”I have a sore throat.”(我喉嚨痛)、”I have a fever.”(我發燒)、”I have a runny nose.”(我流鼻水)。Nice and direct.
J
For pain, there’s another handy pattern — “It hurts when I swallow,” “It hurts when I move my arm.”講到「痛」,還有另一個好用句型——”It hurts when I swallow.”(我一吞東西就痛)、”It hurts when I move my arm.”(我一動手臂就痛)。It hurts when I, plus a verb.It hurts when I 後面接動詞。Super natural, super useful.
M
And they’ll almost always ask, “How long have you had this?” How long have you had this?”(這個狀況多久了?)。That’s present perfect — something that started in the past and is still true now. You just answer with something like “I’ve had it since last night,” same idea Amy used.你只要回答類似 “I’ve had it since last night.”(我從昨晚開始就這樣了),跟 Amy 用的邏輯一樣。
J
To actually get in the door, say “I’d like to schedule an appointment” for a PCP, or just ask “Do you take walk-ins?” for urgent care.真正要進得了門,找 PCP 就說 “I’d like to schedule an appointment.”(我想預約看診);找 urgent care 就直接問 “Do you take walk-ins?”(你們接受免預約嗎?)。Both are everyday, natural phrases.
M
And once you’re seen, if the doctor writes you a prescription, that just means picking it up at the pharmacy afterward — we already covered that whole scene in another episode, so we’re not repeating it today.
J
Right, today’s really about the decision before you even get to the doctor — where to go, and what to ask about the bill.
M
Notice how calm Amy actually stays through the whole call. She doesn’t panic and rush straight to the ER. She checks the symptoms, checks the cost, then picks the right place.
J
That’s really the whole skill — not memorizing every medical word, just knowing the decision: not urgent, not dangerous, urgent care. And always, always ask about the copay before you sit down.
🔁 [DIALOGUE REPLAY]
M
Listen again for one line — “Nope, it’s walk-in. Way faster and cheaper than the ER.”重聽一次,注意這句——”Nope, it’s walk-in. Way faster and cheaper than the ER.”(不用,直接上門就行,比急診快又便宜很多)。Honestly, that single sentence is the whole lesson.
J
So next time you’re not feeling great in the States, don’t panic. Ask yourself “urgent or dangerous?”, then ask about your copay. Try saying it out loud, even just to yourself.
M
Alright, give it a try before your next trip abroad.
J
See you next time on MJ English!
M
Bye, everyone!

Next Time

I want to go to the hospital.

I think I should go to urgent care.

How much will it cost?

Do you take my insurance? What’s my copay?

My throat is very pain.

I have a sore throat. It hurts when I swallow.

I have a sore throat,要嘛用 it hurts when 句型,兩個都比硬翻的 very pain 自然又正確。
